Fine over Livingston worker’s forklift injuries accident
A Livingston-based construction supply company has been fined after a worker was seriously injured when he was run over twice by a forklift truck.

Livingston Sheriff Court heard a SIG Trading Ltd forklift driver was moving a large metal coil with a forklift truck inside the company’s warehouse on Houston Industrial Estate.

The court was told the metal coil was loaded and as the forks were in the raised position, the driver’s vision was restricted. The driver was unaware two other employees were in the warehouse but as he drove the truck forward two or three metres, he became aware of a colleague shouting and indicating to him to reverse.

Unaware he had already hit one of the workers knocking him to the ground and running over his right leg, he then reversed the truck over his co-worker’s leg once again.

At Livingston Sheriff Court today (22 August 2011) SIG Trading Limited, of Hillsborough Works, Langsett Road,Sheffield, pleaded guilty to breaching Section 2 of the Health and Safety at Work etc Act and was fined £36,000.

Middlesbrough firm fined after worker hurt by 600kg bag of fertiliser
A North East fertiliser company has been fined after a worker was seriously injured when a 600kg bag of ammonium nitrate fell on him.

Robert Dearlove, 32, was part of a team clearing up a spillage after several bags of ammonium nitrate fell from a stack at a Fertiliser Solutions Ltd warehouse at Dawson’s Wharf, in Middlesbrough.

The men were working near the remaining stack of bags which was stacked three high and had noticed that two bags on the third layer were leaning to one side. After Mr Dearlove hooked up the last of the fallen bags to the forklift truck, he turned around with his back to the stack.

One of the bags from the third layer fell and struck him on the back, forcing him over and causing him to strike his head on the floor. He suffered a serious back injury, a fractured right thigh and ligament damage to his left leg.

Fertiliser Solutions Ltd, of Dawson’s Wharf, Middlesbrough was fined a total of £10,500 and ordered to pay £12,411.45 in costs after pleading guilty to two offences, one breach of the Health and Safety at Work etc Act 1974 and one breach of the Management of Health and Safety at Work Regulations 1999.

Barnet bread company fined after man dies from fall
A Barnet-based wholesaler has been fined after a man died of injuries sustained when he fell from a stepladder.

On the morning of 22 April 2009 Mr Rocco Carofalo, who was employed as a casual handyman at Ovenpride’s Finchley Rd Bakery, was asked to build shelving in the storeroom by the site manager.

During the morning Mr Carofalo had been seen standing on a stepladder while working on the shelving using tools and materials provided by Ovenpride.

At around midday he was found lying on the floor bleeding from a severe head wound, with the stepladder beside him. He died as a result of his injuries on 23 June.

Ovenpride Wholesale Ltd, of Empire Way, Brent pleaded guilty to breaching Section 2(1) Health & Safety at Work etc Act 1974. It was fined a total of £1 .

Amjad Mahmood, 57, of Donnington Road, Brent pleaded guilty to also breaching Section 2(1) of the Health & Safety at Work etc Act 1974. He was fined a total of £300 and ordered to pay costs of £200
