A Staffordshire engineering firm has been fined £40,000 after a worker was crushed to death underneath a plant vehicle.

Overview
IOSH reported that Mark Palmer, a 46-year-old employee of Hydraline Engineering Ltd, was working under the arms of a wheeled loader when the hydraulic system lost pressure, causing the loading arm to fall and crush him against the vehicle’s frame.

Outcome
Hydraline, of Unit 33 Martindale, Cannock, was prosecuted by the Health and Safety Executive (HSE) over the incident at Wells Farm, Bradley, Stafford, on March 7, 2008.

The company admitted breaching the Management of Health and Safety at Work Regulations 1999 and the Health and Safety at Work etc Act 1974, and was ordered to pay £20,000 costs on top of the fine.
